Certification Application

For a detailed description of the certification process, download the Certified Member Mediator Application (PDF). You can submit your materials electronically to and mail via USPS to us your original signed and notarized “Affidavit of Compliance” or send in all materials and payment via USPS – see page 2 of the Certified Member Mediator Application (PDF) – to the address below. We suggest that you keep a full copy of your application for your records.


You can make your payment online, or, if you prefer to send a check via USPS, please make your check payable to WMA, and please note on the check if MWA Certification Application you are joining, and send in your check to:
